I will not be accepting Anthem at this time.

Out-of-network options: ​If I am not in network with your insurance company, I can provide a superbill if you have out-of-network benefits. This means you would initially pay out-of-pocket and be reimbursed the out-of-network rate after submitting the superbill to insurance. I also may be able to offer sliding scale for those who might be eligible.
